Photography/publicity permissions
Occasionally Bransgore Youth Football Club would like to publicise the club and its players on our website www.bransgoreyouthfc.co.uk. To do this we require your permission for your child's full name and/or image(s) to be used.

I give permission for my child to have their photograph taken

I give permission for my child's full name and Image(s) to be used on the club website & in the media

I give permission only for my child's image(s) to be used on the club website & in the media

I understand that these images may be used for club publicity purposes

I confirm that I have answered all the above to the best of my knowledge.

Once you have submitted your form please then proceed with payment for the annual membership fee of £100.

Any match or training clothing that is supplied by the club will need to be returned once the player leaves or if the club require it back at any other time, (lost items can be ordered through us at cost.)

If you are NEW to the club please upload a copy of the players passport or birth certificate
